Access Requests

Form: Request Access to Tableau or Mixpanel

Reporting and Insights

Form: QR Code, Branded Short Link, or Tracking Codes
Request a QR code or short link (Rebrandly or Branch) that includes tracking for campaign performance and Mixpanel data integration.
Form: New Dashboard / Dashboard Update / Data Request
Request a new or updated dashboard in Tableau or Mixpanel, or ask for a one-time data report. Use this form for audience analysis or data visualization needs.
Form: New Audience Creation
Request the creation of a specific audience segment to target certain patient or user groups for marketing or outreach campaigns.

Data Collection

Form: New Data Collection Tracking
Request tracking setup to capture new data using Mixpanel, client-side or server-side analytics, or marketing pixels.

Report an issue with analytics tracking, such as Mixpanel events not firing, duplicate or missing tags, incorrect page or app data, or other client-side or server-side tracking issues.

Data Engineering

Report and issue with data pipelines or tables, such as dashboards not refreshing, missing or delayed data, or tables that have stopped updating.

Report missing, incorrect, or outdated information in Yext or Google My Business listings.
